天麻と沢瀉の違いは、生薬の源、性質、風味と経絡、効能、適応、注意などに反映される。
1.生薬源：天麻はテンナンショウ科のテンナンショウ（Tennantia heterophylla）またはテンナンショウ（Tennantia northeasternis）の塊茎を乾燥した漢方薬で、沢瀉はテンナンショウ科の沢瀉（Semen xiaceae）の塊茎を乾燥した漢方薬である。
2.味と経絡：天南星は味が苦・辛で、性質が温、毒があり、肺・肝・脾の経絡に適応する；蝉霞は味が辛で、性質が温、毒があり、脾・胃・肺の経絡に適応する。
3.効能：天南星はしこりや腫れを散らす作用があり、半夏は湿を乾かして痰を解消し（湿を乾かして痰を解消して湿を排出する）、反撥を鎮めて嘔吐を止め（胃気の上行を防いで嘔吐を止める）、斑を鎮めて節を散らす（体の節やしこりを解消する）作用がある。
4. Indications: Tiannanxing is used externally to treat carbuncles and swellings, snake and insect bites; Bianxia is used in the treatment of damp phlegm and cold phlegm, wind phlegm and vertigo, phlegm-convulsive headache (headache caused by phlegm obstructing the qi), coughing and wheezing with excessive phlegm and phlegm-drinking, phlegm-drinking and vertigo palpitations (vertigo and palpitations due to phlegm-drinking and evil qi upward), chest and epigastric constipation, vomiting and regurgitating, prune-bearing qi (a condition similar to pharyngeal hypersensitivity), as well as the treatment of carbuncles and swellings, and phlegm nuclei externally.
